There are two
types of forms provided below. One is in PDF, which can be printed and filled out
manually and forwarded to the Association by fax, email or mail.
The other is an Excel file. You will be required to save it for your own records before emailing it to firstname.lastname@example.org.
Both Reporting Forms have highlighted areas which need to be completed. Those
completing the form within Excel only need to enter data in
the highlighted cells. There are
formulae embedded in the excel file which will calculate the points based on the input

There are two examples of PDF files.
The first file is a list of possible activities for 2016 which any member could partake in.
The second file is a completed Excel form based on the list of activities for 2016.
